Complete, sign and mail application to: MTB50, 103 E.Hamilton Ave., State College, PA 16801 (or hand deliver to the mail slot at the same address.)

NOTE: Incomplete applications will be returned. No refunds. Teams changing rotation order, runners, category or division after September 25th will be assessed an additional $25 fee. Absolutely no changes can be made after Monday, October 11th.
